To avoid missed appointment fees speak directly with a staff member, 24 hours before your appointment during our regular business hours (Mon-Thurs). Cancellation left by voicemail, answering machine, text message, or email is not accepted. Your appointment is not considered canceled until a staff member has confirmed the cancellation.
Please Note: All Health History Forms Must be Fully Completed & Submitted or Delivered to the office 24 hours before your appointment or you will be rescheduled.
